RESUMO

Abstract Angiomyolipomas (AMLs) are rare benign tumors derived from mesenchymal tissue and composed of varying degrees of adipose tissue, muscle and blood vessels. Renal AMLs (RAMLs) are the result of a sporadic event, and, in most of cases, the diagnosis is usually incidental, but hemorrhage and shock may be present. During pregnancy, the size of AMLs may increase and they may rupture, probably due to the high expression of hormone receptors, and the increase in maternal circulation and abdominal pressure. The authors present a case of a woman with ruptured RAML submitted to urgent endovascular treatment four days after giving birth by cesarean section.


Resumo Angiomiolipomas (AMLs) são tumores benignos raros derivados do tecido mesenquimal, compostos em graus variados de tecido adiposo, muscular e de vasos sanguíneos. Os AMLs renais (AMLRs) resultam de um evento esporádico e, na maioria dos casos, o diagnóstico costuma ser fortuito, mas quadros de hemorragia e choque podem estar presentes. Durante a gestação, os AMLs podem aumentar de tamanho e romper, provavelmente pela altaexpressãodereceptoreshormonais,epeloaumentodacirculaçãomaternaedapressão abdominal. Os autores apresentam um caso de uma paciente com AMLR roto submetida a tratamento endovascular de urgência no quarto dia pós-operatório de uma cesariana.

RESUMO

OBJECTIVE: To evaluate the safety and efficacy of transarterial nephrectomy, i.e., complete renal artery embolization, as an alternative to surgical nephrectomy. MATERIALS AND METHODS: This retrospective study included 11 patients who underwent transarterial nephrectomy due to a high risk of surgical nephrectomy or their refusal to undergo surgery during the period from April 2002 to February 2013. Medical records and radiographic images were reviewed retrospectively to collect information regarding underlying etiologies, clinical presentations and embolization outcomes. RESULTS: The underlying etiologies for transarterial nephrectomy included recurrent hematuria (chronic transplant rejection [n = 3], arteriovenous malformation or fistula [n = 3], angiomyolipoma [n = 1], or end-stage renal disease [n = 1]), inoperable renal or ureteral injury (n = 2), and ectopic kidney with urinary incontinence (n = 1). The technical success rate was 100%, while clinical success was achieved in eight patients (72.7%). Subsequent surgical nephrectomy was required for three patients due to an incomplete nephrectomy effect (n = 2) or necrotic pyelonephritis (n = 1). Procedure-related complications were post-infarction syndrome in one patient and necrotic pyelonephritis in another patient. Of four patients with follow-up CT, four showed renal atrophy and two showed partial renal enhancement. No patient developed a procedure-related hypertension. CONCLUSION: Transarterial nephrectomy may be a safe and effective alternative to surgical nephrectomy in patients with high operative risks.

INTRODUCCION: Los angiomiolipomas renales son tumores benignos infrecuentes. Tanto la observación periódica como el tratamiento quirúrgico y la embolización arterial selectiva son modalidades válidas para el manejo de dichas lesiones, teniendo en cuenta su tamaño y la sintomatología del paciente en cuestión. OBJETIVOS: Presentar un caso de angiomiolipoma renal, así como el tratamiento utilizado. PRESENTACION DEL CASO: Paciente masculino de 65 años de edad, quien asistió a consulta por dolor lumbo-abdominal intenso del lado derecho. En el ultrasonido se constato imagen tumoral hiperecogénica con densidad grasa en la cara anterior y parte media del riñón derecho. La lesión fue corroborada mediante tomografía axial computarizada, y presentaba las características típicas del angiomiolipoma, con aproximadamente siete centímetros de diámetro. Se realizó la embolización superselectiva mediante una técnica mixta de gelfoam y alcohol absoluto. El paciente no presentó ninguna complicación. Se ha mantenido asintomático y en la evolución tomográfica, realizada a los seis meses de la embolización, se constató la disminución del tamaño de la lesión y de su porción angiomiogénica. CONCLUSIONES: La embolización superselectiva de los angiomiolipomas renales sintomáticos constituye una opción de tratamiento electivo segura y mínimamente invasiva, que permite la conservación de la unidad renal


INTRODUCTION: The renal angiomyolipomas are uncommon benign tumors. As much the periodical observation like a surgical treatment as the selective arterial embolization are valid modalities for the management of such lesions, taking into account its size and the symptoms of patient as regards. OBJECTIVES: To present a case of renal angiomyolipoma, as well as the treatment applied. CASE PRESENTATION: A male patient aged 65 who came to consultation due to intensive lumboabdominal pain in right side. In ultrasound (US) it was possible to verify a hyperechogenic tumor image with fat density in the anterior face and in the middle part of right kidney. The lesion was corroborated by computed axial tomography with the typical features of an angiomyolipoma of approximately seven centimeters of diameter. A Overselective embolization was carried out by means of a mixed technique of Gelfoam and absolute alcohol. There was not complication and the patient has remained asymptomatic and in the tomography course carried out at 6 months of embolization, there was a decrease in the lesion size and of its angiomyogenic. CONCLUSIONS: The Overselective embolization of symptomatic renal angiomyolipomas is a safe and minimally invasive option of the elective treatment allowing the conservation of renal unit

RESUMO

Therapeutic embolization is defined as the voluntary occlusion of one or several vessels, and this is achieved by inserting material into the lumen to obtain transient or permanent thrombosis in the downstream vascular bed. There are a number of indications for this approach in urological practice, in particular for the patients with parenchymatous or vascular kidney disease. In this review, we present the different embolization techniques and the principally employed occluding agents, and then we present the principal clinical indications and we discuss other pathologies that may benefit from this non-invasive therapy. The complications, side effects and main precautions associated with this approach are also described.

RESUMO

OBJECTIVE: To evaluate the therapeutic efficacy of a new liquid embolic materi-al,Embol, in embolization of the renal artery. MATERIALS AND METHODS: Embol is a new embolic material obtained by partial hydrolysis of polyvinyl acetate mixed in absolute ethanol and Iopromide 370 and manufactured by Schering Korea, Kyonggido, Korea. Six patients who underwent embolization of the renal artery using Embol were evaluated. Four were male and two were female and their ages ranged from 11 to 70 (mean, 53) years. Clinical and radiologic diagnoses referred for renal artery embolization were renal cell carcinoma (n = 3), renal angiomyolipoma (n = 2) and pseudoaneurysm of the renal artery (n = 1). After selective renal angiography, Embol was injected through various catheters, either with or without a balloon occlusion catheter. Changes in symptoms and blood chemistry which may have been related to renal artery embolization with Embol were analyzed. RESULTS: The six patients showed immediate total occlusion of their renal vas-cular lesions. One of the three in whom renal cell carcinoma was embolized with Embol underwent radical nephrectomy, and the specimen thus obtained revealed 40% tumor necrosis. In the two patients with angiomyolipomas, the tumors decreased in size and abdominal pain subsided. Bleeding from pseudoaneurysm of the renal artery was successfully controlled. Four patients showed symptoms of post-embolization syndrome, and one of these also showed increased levels of blood urea nitrogen and creatinine. One patient experienced transient hyperten-sion. CONCLUSION: Embol is easy to use, its radiopacity is adequate and it is a safe and effective embolic material which provides immediate and total occlusion of renal vascular lesions.

RESUMO

En el período de marzo de 1985 a mayo de 1996, fueron tratados 26 pacientes del sexo femenino y uno del sexo masculino con diagnóstico de angiomiolipoma renal. Veintitres pacientes presentaban compromiso renal unilateral (85 por ciento) y, de las 4 pacientes con compromiso bilateral (15 por ciento), tres tenían asociación con esclerosis tuberculosa (10 por ciento). Debido a complicaciones, tres tenían asociación con esclerosis tuberculosa (10 por ciento). Debido a complicaciones, tres pacientes con compromiso bilateral y uno con unilateral necesitaron tratamiento quirúrgico. Los pacientes están en seguimiento, por un período mediano de 38 meses
